fix(skills): constrain artifact writes to managed directories

The skill didn't specify exhaustive write destinations, so Claude
would default to writing plan/research docs to docs/plans/ in
whatever directory the user happened to be in. Now the skill
declares exactly three writable locations (runstate, library,
manuscripts) and removes the legacy docs/plans/ fallback read.
