feat(skill): add Phase 4.9 agent readiness review loop to SKILL.md

- Insert PHASE 4.9 section between Phase 4.8 and Phase 5
- Steps 4.9a-d: check availability, run reviewer, implement fixes, termination check
- Severity-gated loop: max 2 passes, zero Blockers/Frictions to pass
- Phase gate with Pass/Warn/Degrade verdicts
- Graceful skip only if compound-engineering plugin unavailable
- Update frontmatter version from 1.1.0 to 1.2.0
- Update phase map diagram with Phase 4.9 (5-10m budget)
- Update phase count from 8 to 9 mandatory phases
- Update total time from 45-85 to 50-95 minutes
