Completed. I read `CODEX_REVIEW.md`, walked the P0/P1 items, and found no item that matched the approved autonomous safe-fix list closely enough to edit safely.

I only added [CHANGES.md](/Users/stevestudio2/Projects/secrets-manager/CHANGES.md), documenting:
- `## Applied`: none
- `## Deferred (needs Steve)`: all P0/P1 issues with file lines and deferral reasons

I did not run tests, install packages, touch git, or modify any source files.
