Implemented the safe fix and wrote [CHANGES.md](/Users/stevestudio2/Projects/the-ai-factory/CHANGES.md).

Changed:
- [src/llm.js](/Users/stevestudio2/Projects/the-ai-factory/src/llm.js:10): removed the hardcoded `/Users/.../claude` fallback and now defaults to `claude`.
- [.env.example](/Users/stevestudio2/Projects/the-ai-factory/.env.example:15): added `CLAUDE_CLI=claude` as a placeholder/config knob.

Deferred items are documented in `CHANGES.md` with severity, file lines, issue, and reason. I did not run tests, install packages, commit, push, or touch git.
