Implemented the safe fixes from `CODEX_REVIEW.md`.

Changed:
- `scripts/deploy-direct.sh`
- `scripts/deploy-pastdoor.sh`
- `.env.example`
- Added `CHANGES.md`

What changed: hardcoded GoDaddy credential fallbacks were removed, both deploy scripts now fail fast if required GoDaddy env vars are missing, and `.env.example` now contains placeholder-only env var names.

I did not run tests, install packages, or touch git, per instructions. `CHANGES.md` documents applied fixes and deferred items under 500 words.
